C4CC Newsletter – Issue 29
In our latest newsletter, we focus on two key areas of the Coalition’s ‘Three Cs’. Progress in Personalised Care and Support Planning is a practical tool for people to assess their progress in implementing care and support planning and create an action plan to move towards excellence in this area. The LEHC programme holds true to the values of the Coalition, and models co-production, with people with lived experience involved in both the design and delivery.

This edition of our newsletter comes from Professor Nigel Mathers, RCGP’s Honorary Secretary and C4CC Co-chair.
